Are you recovering from… surgery, a stroke, a heart attack or an injury?

St. Lawrence County Public Health Department’s home care program can help you recover from illness and injury at home!

St. Lawrence County residents of all ages can benefit from home health care provided by the St. Lawrence County Public Health Department.
 

How can I be sure this is a quality service?

Since 1936 the St. Lawrence County Public Health has been serving the residents of St. Lawrence County. The Department’s Certified Home Care Agency meets or exceeds federal and state regulations. Our highly educated and experienced Health Team works closely with physicians to provide a full range of health care services in the home.
 

Am I eligible?

If you temporarily reside or are a resident of St. Lawrence County you may be eligible for home health care services. In addition, you must:

• have a physician approved plan of care
• require skilled care due to a medical condition
• have a safe home environment and can be safely cared for at home

How do I apply for home care?

The first step is to ask your physician, medical practitioner, hospital, or community agency for a referral. You may also call our Canton office to find out more information.

Is there a cost?

If you receive Medicare, Medicaid, or have Private Health Insurance, fees may be covered in part or in full. For service not covered by insurance, charges are based on your income and a sliding fee scale is available.
 

When are you available?

As a New York State Certified Home Health Care Agency, we are available to our patients 7 days a week, 24 hours a day.
 

What services do you offer?

We provide a wide range of services to assist you while recovering at home. Services include:

• Skilled Nursing Care
  Our professional nursing staff provides high quality care including teaching, injections, medication management, wound care, intravenous (IV) therapy and catheter care.
   
• Home Health Aides
  Our aides are specially trained to provide general personal care including bathing, dressing, toileting, and exercise programs.
   
• Therapy Services
