Children With Special Health Care Needs Program

Program Goal

Provide resources and referrals for children in the community to ensure access to health care, insurance, information and support.
 

Eligibility

Children from birth to 21 years who have (or may have) a serious or chronic physical, developmental, behavioral or emotional condition.
 

Description

This program provides assistance in locating a physician or other health care provider to ensure each child has a medical home. The program may also provide assistance in obtaining insurance and/or other programs that help to cover health care costs – i.e. Medicaid, Child Health Plus. Supportive measures as well as medical information may be provided.
