Are you looking for an alternative to Nursing Home care for your loved one?"

The Long Term Home Health Care Program may be the answer.

Working in conjunction with the St. Lawrence County Department of Social Services and other community agencies, we may be able to tailor a care program for your loved one's individual needs.
 

Is my loved one eligible?

If your loved one is a resident of St. Lawrence County he or she may be eligible for Long Term Home Health Care Program. In addition, the patient must qualify for:

  • nursing home level care
  • have a physician approved plan of care
  • reside in his or her own home or that of a responsible care giver
  • require care due to a medical condition
  • have a safe home environment including a working telephone and can be cared for safely in the home.

It is also important that the patient have someone who is willing to assume responsibility to serve as his or her caretaker.
 

How do we apply for Long-term Care?

The first step is to ask your physician, medical practitioner, hospital, or community agency for a referral. You can also call the Program at (315) 386-2325.
 

Is there a cost?

Long Term Home Health Care Program costs are covered by Medicaid.
 

How do I know what's the best plan of care for my loved one?

Our nursing staff will work with you to determine your needs. A plan of care will be developed to keep you or your loved one safely in their home setting.
 

Who will take care of my loved one?

Professional members of our staff will come to your home to help you meet your loved one's day- to-day needs.
 

What services do you offer?

A wide-range of services are available and include:

  • Skilled Nursing Care
    Our professional nursing staff will work with you to coordinate the care needed. In edition, the nurses provide injections, teaching, wound care, intravenous (IV) therapy and catheter care should the need for these services arise while enrolled in the program.
     
  • Home Health and Personal Care Aides
    Our aides are specially trained to provide general personal care including bathing, dressing, toileting, meal preparation, exercise programs, and light housekeeping.
     
  • Therapy Services
    · Occupational Therapy
    · Speech Therapy
    · Respiratory Therapy
    · Physical Therapy
    · Nutrition

    · Social Work
     
  • Other Services
    · Homemaker (housekeeping, grocery shopping, laundry)
